Health Care Home (HCH)
The Centre has been involved since the 2011 Primary Care Thought Leaders conference in Washington in the development of Australia’s Health Care Home model. Prof Jackson was a member of the Primary Health Care Advisory Group (PHCAG) which described Australia’s HCH model, and the Centre works closely with partners in NZ, the UK and the USA in HCH development, implementation and evaluation. The PCPIT provides an establishment tool for practices and PHNs interested in practice HCH preparation.
